Rafting Forms

Before participating in a Mountain Whitewater rafting trip every rafter is required to sign an Acknowledgement of risk Waiver

There are two Group Organizer forms: A Sign-up sheet and an Organizer Check sheet. These are provided to help the person who’s doing the organizing and planning for a group rafting trip (group organizer rafts for free). Group organizers will not be required to use these forms or turn them into Mountain Whitewater staff.

Photography Order Forms can be filled out ahead of time if you would like to purchase the photos from your trip, these order forms are also available from the office staff at Mountain Whitewater during your visit. If you forgot to purchase photos, please fill out a form and send it to the office staff at Mountain Whitewater so the photo flash drive can be mailed, we will keep the photos on file for seven years.

The Essential Eligibility Criteria describes eleven eligibility requirements rafters need to meet before taking a rafting trip with Mountain Whitewater.
